The medical conditions screen is simply completed by checking each box that applies to your situation. These same checkboxes will appear on the HealthProfiles report.
What if your condition is NOT listed here? Don't worry. We thought of that too. There's a place on the Other Conditions screen where you can describe the condition that was not listed. That space can also be used to describe temporary conditions such as a broken bone. Of course all of the "other conditions" will also be printed on your health profile.
