Upload Your PDF Documents

| · | Upload password protected PDF files of all your health related data to your own secure folder for Internet access from any location with an Internet connection.
|
| · | Upload password protected PDF documents such as; Passports, Birth Certificates, Insurance papers, and any other important documents.
|
| · | Perform unlimited viewing and printing of any of your documents from any Internet connected computer, anywhere in the world. This includes your doctor's office, hospital, emergency room or wherever the need arises. There is no additional cost for downloading or viewing your health profile or other data. Your ID and password will be needed for access.

| · | Perform unlimited updating and refreshing of any previously uploaded documents. There is no additional cost for this service.
|
| · | Use Health Profiles to create or convert other medical records or reports for secure storage and access. Convert data to an acceptable password-protected and encrypted PDF format. This format ensures that your data is not readable nor accessible without your ID and password. See Printing Health Reports to PDF Format

| 1. | Every document must be in PDF format.
|
| 2. | Every PDF document must be password protected (from viewing), and encrypted using the Health Profiles Create PDF Files option. This ensures your data is secure and safe.
|
| 3. | Only documents of reasonable size may be kept. So you can not upload books, backup files, or any non-PDF files. While we do not impose a strict limit on the number of files you may upload, we do watch for abuses, and reserve the right to terminate the subscription of anyone who abuses the program. Complete terms of the subscription are available prior to sign up.

| 1. | Select the Register for Upload Subscription option on the Help and Registration Menu or the Upload Your PDF Documents option from the Portable Health Profiles Menu

| 2. | If you have an Internet connection, click the Enroll in Health Profile Upload Button
|
| 3. | Complete the on-line enrollment form and select either the 6 month or one year subscription option.
|
| 4. | Once you have enrolled, you will receive an email notification with instructions on usage including your login and password.
|
| 5. | If you do not have immediate access to the Internet, you can log into www.MyHealthProfiles.com to enroll. Once you are logged into MyHealthProfiles.com, click on Order Now and scroll down to Subscribe To Health Profiles Uploads and click on the Begin the Enrollment Process button.
