Add or Update Surgeries & Procedures

The Add/Update Window: this window display the contents of a single record. It is invoked when you click on the INSERT button. This window appears if you are adding a new record or updating a previously saved record. The functions in Add/Update Mode are: OK to save data that you enter, CANCEL to not save the data and HELP.
 
surgerydata  
Enter data in each field. Use the TAB key to move from field to field. Mouse click the TAB to change views and data.

TAB 1) GENERAL INFORMATION

Person's Name
   
The name of the individual will be automatically displayed to confirm that you are entering data for the correct person.

Date of Surgery    
Enter the date or use the calendar icon to pick a date.

Surgical Procedure    
Enter the name of the surgery or procedure.

In Patient Check Box
Click the box to indicate an In-Patient procedure, if not checked, the procedure will default to an Out-Patient one.

Surgical Procedure    
Enter the name of the surgery or procedure

Hospital Name
   
Enter the name of the location of the surgery, hospital, clinic, doctor's office, etc.

Affliction Name
   
Enter the name of the illness, injury, or disease that prompted the need for the surgical procedure.

Out Come
   
Enter the result of the surgery

Surgical Procedure    
Enter the name of the surgery or procedure.

TAB 2) REASON FOR SURGERY
   
If you choose, you can use this area to log more information related to the surgery. It may be helpful to record complications, medications and or drugs used including anesthetics that worked well or were problems.

Accept data and closes the window
Cancel Button
Cancel operation; restores original information
Help Button
