Medical Equipment

Enter the name and type of any medical equipment or devices you may use. Include the name and any important information about the equipment such as the make and model or serial number. Include current settings, if applicable. Include relevant and related parts. For example, the type of mask you may use with oxygen. Be as clear and thorough as possible.

My Equipment is Not Listed - What Do I Do?

You can add equipment on the OTHER DEVICE box or add equipment and/or devices on Tab 5, Allergies and Conditions. Click on the tab and enter information in the free-form box under Conditions.
