Medical Conditions

Use your mouse and the left-click button to place a check mark on any known conditions you may have. The left mouse button acts as a toggle to check or uncheck any box. If you have no known conditions, check that box.

Abbreviated Names

Some of the conditions have abbreviated names. To see the complete name, hold the mouse pointer over the short name, and the full or alternate name will appear in a pop-up box.

Want To Add More Details?

Use Tab 5, Conditions to add more descriptive details. The Conditions area lets you add free-form information to clarify or elaborate on a condition.

My Condition is Not Listed - What Do I Do?

You can add a condition on Tab 5, Allergies and Conditions. Click on the tab and enter information in the free-form Conditions box.
