Allergies & Conditions

Enter any allergies you may have to food or drugs or animals. You may want to include the type of allergic reaction you have especially if you have a severe reaction to any specific allergen. If you have pollen or other allergies, enter those in Other Allergies.

What are Other Conditions? and How Should I Use It?

This Other Conditions box is a free-form data entry area, where you can include things not designated in other parts of the Profile. For instance, you can enter medical conditions not previously identified, additional medical equipment, helpful explanations or whatever information that you deem important.
